VET CONSENT
In order to comply with the Veterinary Surgeons Act 1966 & Veterinary Surgeons (Exemptions) Order 2015, I am required to gain consent from your vet, for me to provide Physiotherapy, prior to our first session. This is to ensure I am fully informed of your dog’s clinical conditions and medical history and any interventions that might be contradicted.
Once you’ve made contact with me to book an initial assessment, I will send over my vet consent form to provide to your vet to complete, which will need to be returned to me before our first session. Alternatively, I am happy to contact your vet directly to gain consent, if you provide me with their details.
*Please note: Vet consent is required for all clinical cases, but is not needed for maintenance sessions on healthy dogs.
